A quick guide to simplify your sign up with Labworks USA

We will explain all the information you will need on hand to make signing up simple and quick.

1. Company Information

Here is what you will need to complete this page:

If you are NOT an Owner Operator you need to know who your DER is and if they have a valid certificate.

2. Driver Information

Have this information at hand to complete this step easily:

Drug Test

Return-to-Duty (RTD) – Select this if the driver previously tested positive, completed the SAP program and is cleared to take the return to duty test.

Pre-Employment (PE) – This is required of the driver has NOT been in a random program for more than 30 days.

Physical / Medical certificate

A valid DOT Medical certificate must be renewed every 2 years.

If it is expired or is soon to expire select YES. Select NO if it is up to date

3. Labworks Offered Services

Confirm your FMCSA Clearinghouse (CH) status. If you are an Owner Operator and you are registered in the CH you must select the options for Labworks USA to be your C/TPA and mark all the options for Labworks USA to fully manage your CH duties.

Company Policy on Drug and Alcohol Testing

Every Trucking Company that is regulated by the DOT must have a company drug and alcohol testing policy on hand and it must be given to every employee and they must sign it and you must keep a copy on file.

If you do not have an existing Company Policy you can purchase one during sign up. When you have all of this information you are ready to begin.
